Chronic kidney disease (CKD) affects about 10% of adult Australians, resulting in significant morbidity and mortality, placing a large and increasing burden on the healthcare system. Despite its widespread presence in Australia, there are ongoing concerns
that CKD remains under-recognised and under-treated, especially as patients may lose up to 90% of kidney function before symptoms are present.
Primary Care is perfectly placed to identify people with undiagnosed kidney disease and begin early interventions that can slow progression to kidney failure.
The clinical audit focuses on identifying the practice gaps that lead to CKD remaining undiagnosed in people with evidence of chronic kidney disease - eGFR <60mL/min/1.73m2 or Urine ACR >3mg/mmol.
Using clinical data reports through PENCAT, the practice team is supported to understand the gaps, and the problem, along with how to begin systematically coding diagnoses for CKD. The clinical audit requires a minimum of 2 GPs participating at each
practice, with the whole practice team encouraged to be part of the learning process.
Learning outcomes:
- Evaluate current practice systems for identifying and recalling patients with evidence of CKD.
- Analyse practice data using PENCAT to identify patients with evidence of chronic kidney disease.
- Compare practice data against best practice guidelines and Public Health Area data (PHAs).
- Identify 20 patients with evidence of CKD and action Kidney Health Checks for each.
- Develop a whole of practice plan for the early diagnosis of people with CKD clinical indicators using the Kidney Health Check algorithm.
Enrolment requirements:
- Clinical Lead name and email at the practice
- Practice Manager name and email (if different from Clinical Lead)
- Nominee for generating PENCAT reports for the practice & upload to Clinical Audit dashboard
- Minimum of 2x GP names, emails, & RACGP numbers
- Consent for Practice Name and suburb to be included in reports and communications
- Minimum of 3x practice meetings dedicated to the Clinical Audit
